The icon set is called 'umicons' and you can get it here. Go to the icon section and click on the highest rated tab. It should be right there on the first page.

to display the weather, you need an application called 'superkaramba'. With this application, you can run themes on your desktop such as the 'liquid_weather' theme you see in my screenshot. look for the application and the theme here at kde-look.org in the karamba section.
